Description:

The purpose of this Request for Proposals (“RFP”) issued by the Office of Temporary and Disability Assistance (“OTDA”) of the State of New York (“State”) is to solicit proposals from qualified vendors for the establishment of up to three contracts: one contract medical provider to establish a site in Syracuse (with a satellite office in Jefferson county), one contract medical provider to establish a site in Binghamton (with a satellite site in Chemung or Steuben County), and one contract medical provider to establish a site in Poughkeepsie (with a satellite site in Orange County) for the purpose of performing the State’s mandatory medical examinations and ancillary testing of claimants applying for or currently receiving, Social Security Disability or Supplemental Security Income benefits. Offerors may submit proposals for one, two, or all three sites. Offerors wishing to bid upon more than one site must submit a separate administrative, technical and financial proposal for each site bid upon.
