Overview

NYDocSubmit is an app for mobile devices and is available for download in the Apple App Store and Google Play Store. With NYDocSubmit, applicants and recipients of Temporary Assistance (TA), Supplemental Nutrition Assistance Program (SNAP), Home Energy Assistance Program (HEAP), and Medicaid, who live in a participating social services district, can take a picture of their documents and submit them using a mobile device.

There is no cost to download NYDocSubmit, however, downloading or using the app may result in charges to your data plan with your mobile provider. You are solely responsible for any data access, messaging, and other service rates and charges that may incur in connection with your mobile device and use of NYDocSubmit.

Note: If the attempt to submit documents through NYDocSubmit is not successful, or NYDocSubmit is not available in a social services district, the individual is responsible for submitting documentation and should use another method to do so within the timeframe designated by the social services district.

The other methods for submitting documentation to a social services district include:

  1. Visiting the social services district office and providing a hard copy or scanning via a kiosk; if available,
  2. Via fax machine,
  3. Mailing through the U.S. Postal Service, or
  4. In certain counties, submitting via myBenefits.ny.gov in association with an electronic application for TA, SNAP and/or HEAP, a recertification for TA and/or SNAP, or to report a change for TA and/or SNAP.

Note: Additional counties may be added, so check your app store to see if the social services district in your county is participating.

Residents of New York City (NYC) who are applying for or receiving benefits from one of the programs mentioned above may submit documentation to the NYC Human Resources Administration through ACCESS HRA.

What You Need to Know

NYDocSubmit may save you a trip to the local social services district to provide documentation. Now, an individual can take a picture of a document, enter identifying information and submit the document to their social services district, anytime. It’s convenient, quick and easy. You can use NYDocSubmit for:

  • Temporary Assistance
  • SNAP
  • HEAP
  • Medicaid

Note: NYDocSubmit is not monitored for emergencies and should not be used to submit an application for benefits or to submit a periodic report.

Examples of Documents That Can Be Submitted Using NYDocSubmit

  • Age/Identity – Photo ID, birth certificate, U.S. passport, adoption records, hospital records.
  • Citizenship or Current Alien Status – Birth certificate, hospital records, U.S. passport, military service records, naturalization certificate, USCIS documentation, evidence of continuous U.S. residence since prior to 01/01/72.
  • Income – Current wage stubs, pay envelopes, income tax records, current benefit check, current award letter, official correspondence from the source of income.
  • Medical Documentation – Copies of medical bills, health insurance policy, insurance card, Medicare card. Note: Do not submit documents with sensitive information through NYDocSubmit (see information below regarding documents that should not be submitted through NYDocSubmit).
  • Proof of Who Lives in Your Household – Statement from the non-relative landlord, school records, statements from other persons.
  • Residence – Statement from the landlord, current rent receipt, current lease, mortgage records.
  • Resources – Bank records, stock certificate, income tax refund, burial or funeral agreement, vehicle registration or title of ownership, life insurance policy, a statement from the source of payment.
  • Shelter/Utility Expenses – Current rent receipt, current lease, mortgage records, sewer and water bills, fuel bills, non-heating utility bills.
  • Social Security Number/Proof of SSN Application – Social Security card, official correspondence from the Social Security Administration.
  • Other – Any other information requested by the social services district that does not fall into one of the document categories listed above.

Examples of Documents that Should Not be Submitted through NYDocSubmit

NYDocSubmit should not be used to submit initial applications, recertification applications, periodic reports or documents that include sensitive information, such as:

  • Child Protective Services (CPS) case information or to report suspected child abuse or maltreatment,
  • Documents that contain HIV information,
  • Domestic violence information,
  • Addresses that must remain confidential to safeguard any member of an applicant’s or recipient’s household.

Getting Started with the NYDocSubmit Mobile App

  1. You can download the NYDocSubmit mobile app from the Apple App Store or the Google Play Store.
  2. After launching the app, you can select your preferred language from the following list: Arabic, Bengali, Chinese (Traditional), French, English, Haitian Creole, Italian, Korean, Polish, Russian, Spanish, Urdu, and Yiddish.
  3. You will be prompted to read and agree to the “Terms of Use” and the “Disclaimer”.
  4. You will need to use your NY.gov account to log in to NYDocSubmit. If you don’t have a NY.gov account, you can create one.
  5. To submit your document(s), select your social services district from the list, select the program that you are submitting a document for, or select “not sure” if you don’t know, and then select a document category.
  6. Take a picture of the document. You will have the opportunity to take additional pictures if the document has multiple pages, but please only submit one document at a time.
  7. Provide the first and last name, and phone number, for whom the documents are being submitted. You will also need to enter at least one of the following submission details: Social Security Number (SSN), Client Identification Number (CIN), Case Number, or Date of Birth.
  8. After you confirm the submission details, and submit, a confirmation screen will appear following a successful upload. Wait until you receive the confirmation screen before submitting another document or closing the app.

Note: The confirmation screen shows a tracking number to help identify the document that you submitted. You should write it down or take a screen shot to keep for future reference.
